What You'll Do:
Administer skilled nursing care to patients of all ages in their homes, under the supervision of an RN.
Coordinate care with the interdisciplinary team, patients/families, and referring agencies.
Assist the RN in identifying patients' physical, psychosocial, and environmental needs.
Participate in care planning and implementation in accordance with the plan of care.
Provide care utilizing infection control measures (OSHA).
Ensure continuity of care through the delivery of quality patient care.
Communicate effectively with patients/families, team members, and other healthcare professionals.
Monitor assigned cases to ensure compliance with third-party payer requirements.
Demonstrate commitment, professional growth, and competency.
Promote our philosophy and administrative policies to ensure quality of care.
What You'll Need:
Graduate of an accredited school of vocational nursing.
One year of experience as a Licensed Vocational Nurse in a clinical care setting (home health experience preferred).
Strong nursing skills as defined by generally accepted standards of care.
Excellent interpersonal skills.
Reliable transportation (this is a field-based role with occasional meetings at alternate locations).
Current CPR certification and Hepatitis profile.
Ability to work a flexible schedule, including on-call availability.
Ability to travel locally.
Environmental and Working Conditions:
Work in patients' homes in various conditions.
Possible exposure to blood, bodily fluids, and infectious diseases.1
